The goal of the Horizon’s Supportive Housing Program is to end homelessness and to promote quality independent living for individuals with developmental disabilities. Horizon Housing is a housing first permanent supportive housing agency. This is accomplished by focusing on these five points: 1. Move people with disabilities on the streets or in shelters directly into housing. 2. Collaborate with long-term case management providers to provide robust support services. 3.
